The PCC RN Nursing Manager provides clinical and operational leadership to nurse case managers and support staff in the outpatient clinic. This role is accountable for delivering high-quality, patient-centered care coordination, fostering professional growth among team members, tracking and improving performance metrics, and optimizing workflows throughout the Priority Care Clinic. The PCC RN Nursing Manager partners closely with providers, administrative leaders, and interdisciplinary teams to advance patient outcomes, improve access to services, and ensure compliance with all regulatory standards.
Professional Duties
  • Provide direct supervision to Nurse Case Managers, including hiring, onboarding, training, coaching, and conducting performance evaluations.
  • Delivers day-to-day clinical guidance and serve as an escalation resource for complex or high-risk patient cases.
  • Ensure case management practices align with established clinical protocols, quality standards, and applicable regulatory requirements.
  • Oversee daily workflow, staffing allocation, and caseload distribution to ensure adequate coverage and operational efficiency.
  • Monitor, analyze, and report on performance metrics, including patient outcomes, caseload distribution, productivity, and service utilization.
  • Review documentation for accuracy, completeness, and compliance; provide timely feedback and corrective guidance as needed.
  • Participate in leadership meetings and contribute to departmental planning, strategic initiatives, and goal development.
  • Mentor and develop Nurse Case Managers to strengthen clinical expertise, organizational effectiveness, and communication skills.
  • Foster a positive, engaged team culture that supports retention, professional growth, and career development.
  • Serve as a liaison between Nurse Case Managers, providers, and administrative leadership to promote effective communication and collaboration.
  • Partner with leadership to identify operational gaps, recommend process improvements, and support implementation of solutions.
  • Contribute to quality improvement initiatives, audits, and compliance activities related to case management services.
  • Support initiatives that enhance patient experience, improve care coordination, and drive positive clinical outcomes.

Qualifications
  • Current California Registered Nurse (RN) License required. Bachelor of Science in Nursing (BSN) preferred.
  • 2 years of experience in case management required
  • 3-5 years of outpatient clinical experience; prior leadership or supervisory experience preferred.
  • Knowledge of outpatient care coordination and utilization management in a managed care setting.
  • Strong leadership, coaching and team-building skills.
  • BLS required within 60 days of hire.
  • Detail oriented. Ability to reprioritize tasks in order to meet deadlines.
  • Ability to effectively communicate with individuals within all levels of the organization including providers, patients and others.
  • Demonstrates exceptional clinical judgment with strong analytical thinking and effective problem-solving skills.
  • Strong proficiency in electronic health record (EHR) systems and clinical reporting platforms.
  • Current California driver’s license and proof of insurance.
  • At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
