Job Summary
The Certified Home Health Aide is a para-professional member of the home care team who works under the supervision of a Registered Nurse or Therapist and performs various personal care services as necessary to meet the patient’s needs which are provided under a plan of treatment prescribed by a physician.
 

Qualifications

  • High school diploma or equivalent. Completion of CHHA coursework and certification.
  • Current California Certified Home Health Aide (CHHA) license.
  • Minimum of 1 year experience as a CHHA, or CNA.
  • Scope of practice and Title 22 requirements for CHHA.
  • Excellent customer service skills and good communication skills.
  • Works well in team setting. Able to work independently.
  • Detail oriented.
  • BLS required within 60 days of hire.
  • Current California drivers license and proof of insurance.
  • Must possess own vehicle as position requires travel.

Professional Duties

  • Adheres to duties listed within the Aide Care Plan assigned by a Registered Nurse or other skilled professional, communicates services furnished as well as any unmet expectations or requests for deviation from the documented plan in a timely manner to a Registered Nurse or other skilled professional.
  • Takes and records vital signs, including temperature, pulse, respiration and blood pressure when ordered and reports results outside of the documented parameters to a Registered Nurse or other skilled professional.
  • Understanding, observing, documenting and reporting patient status, including any changes to patient condition or elements of body function, to a Registered Nurse or other skilled professional.
  • Understands and complies with basic infection control procedures and policies.
  • Understanding the physical, emotional and developmental needs and ways to work with the patient population served by the organization, including the need for respect for the patient, his/her privacy, his/her property and his/her rights.
  • Demonstrates competency with assigned tasks including, but not limited to, assisting with ambulation and personal hygiene, as ordered.
  • Meets the safety needs of our patients and using equipment safely and properly.
  • Reminds patients of scheduled self-administered medications.
  • Maintains a clean, safe, healthy environment.
  • Knowledge of adequate nutrition and hydration/fluid intake.
  • Completes documentation in a timely manner.
  • Participation in case conference.
  • Perform other duties as assigned.
  • Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience
