The Intense Case Manager (LVN) is a member of a multi-disciplinary team that focuses on managing care for patients identified as high risk, high cost, or complex secondary to inadequately managed behavioral health and substance use needs. The Intense Case Manager (ICM) is responsible for conducting physical and psychological assessments, medication reconciliations, care coordination, care planning, care plan evaluation and monitoring for assigned patients within multiple environments.
Professional Duties
  • Perform physical assessment for assigned patients to determine medical needs and care coordination needs in the patient's preferred clinical setting.
  • Follow up with the patient and patient's care team to ensure compliance with the established care plan.
  • Serve as a liaison between the patient, patient's family member and healthcare professionals involved in the patient's care.
  • Assist with end of life care conversation and Durable Power of Attorney (DPOA)/Physician Orders for Life-Sustaining Treatment (POLST) and facilitate Palliative Care and/or Hospice referrals as needed.
  • Determine when it is appropriate to involve community protective services agencies and mandated reporting and follow established procedures.
  • Serve as a liaison between the patient, the patient's family, the Interdisciplinary Care Team (IDCT), and other individuals involved in the patient's care. Ensure that necessary information is shared to facilitate well informed healthcare decisions and plan of care goal attainability.
  • Connect patient's with resources that support the patient's care needs for those who lack a support system, lack appropriate resources, or lack understanding of medical benefits.
  • Utilize independent judgment and clinical skills to assess patient needs and coordinate patient's care.
  • Develop a biopsychosocial work assessment based on the patient's specific medical, psychological, financial, and other assessed needs.
  • Create an individualized care plan for each patient within 5 days of opening case.
  • Evaluate and document patient acuity within 3 days of opening case and continuously evaluate patient acuity with every interaction.
  • Inform management of any issues/concerns and situations that fall outside of scope of practice.
  • Perform other duties as assigned.
  • Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience

Qualifications
  • Graduate of an accredited nursing program.
  • Current California Licensed Vocational Nurse (LVN) license.
  • 2 years of nursing experience in an acute care setting.
  • Experience in Behavioral Health and/or experience with substance use disorders, preferred.
  • Understanding of varied diagnostic and invasive procedures.
  • Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors and others.
  • Attention to detail. Ability to manage time effectively and prioritize tasks to meet established deadlines.
  • BLS required within 60 days of hire.
  • Current California driver's license and proof of auto insurance.
  • At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
